Today, open surgery has almost entirely been replaced by minimally invasive stone treatments. The two most commonly used methods are:

  • Mini PCNL (Mini Percutaneous Nephrolithotomy)
  • RIRS (Retrograde Intrarenal Surgery)

Both methods offer high success rates for breaking and removing kidney stones, but their techniques and patient suitability criteria differ.

What Is Mini PCNL? How Is It Performed?

Mini PCNL is a stone removal procedure performed through a skin incision smaller than 1 cm to access the kidney. It is called “mini” because the access tract is smaller than in traditional PCNL.

In this technique, the kidney is reached from the flank region, and stones are directly visualized, fragmented, and removed using endoscopic instruments. It provides high success rates even for large and hard stones.

  • Less bleeding and a smaller incision compared to standard PCNL

It is typically preferred for stones larger than 2 cm, hard stones, or multiple stones.

What Is RIRS? How Is It Performed?

RIRS is a method performed by entering the urinary system through natural pathways. A flexible endoscope is advanced through the ureter into the kidney, where the stone is fragmented with a laser and turned into dust to allow spontaneous passage.

There is no skin incision, and no external intervention is needed.

For this reason, RIRS is known as one of the most comfortable stone treatment methods.

Advantages of RIRS:

  • No skin incision
  • Very low risk of bleeding
  • Faster recovery and discharge
  • Highly repeatable
  • Can be used even in narrow anatomical structures

It is generally preferred for kidney stones smaller than 2 cm.
If the stone is very large, more than one session may be required.

RIRS is preferred for:

  • Small–medium stones
  • Patients with high bleeding risk
  • Patients on blood thinners (with physician approval)
  • Patients who do not want a surgical scar

Important Considerations in Both Methods

  • Pre-treatment stone analysis and imaging are crucial
  • To prevent recurrence, lifestyle modification must follow treatment
  • Chemical stone analysis and metabolic evaluation are recommended
  • Water intake, dietary regulation, and follow-up must continue

Many patients believe “it won’t happen again” after passing a stone.
However, kidney stones tend to recur, and if the prevention–follow-up protocol is not applied correctly, the likelihood of recurrence is high.
